Pine County provides case management services for eligible residents diagnosed with a severe and persistent mental illness. Case managers help individuals access essential resources such as education, healthcare, mental health support, social services, and job assistance, along with other services related to their mental health needs. They assist with completing and monitoring assessments and support plans, referring individuals to necessary services, coordinating care to ensure effective delivery, and regularly reviewing and adjusting services to meet ongoing needs.

Our Partial Hospital Programs help adults and teens stay out of the hospital and get the support they need to heal at home. Our two-to four-week partial programs are directed by a psychiatrist in a group therapy environment. Our Day Treatment Programs are a great fit for individuals looking to improve their mental health and daily functioning in addition to their ongoing outpatient services. Our adolescent programs serve teenagers ages 13-18 and in high school. Our adult programs serve adults 18+.

Children's Mental Health Case Management services are voluntary and may be accepted or refused by the parents. The case manager can assist in the coordination of a comprehensive mental health assessment, if one is not currently available, to determine the child's mental health needs.

Pine County CMH can work with families through an “assessment period” where they will help families access diagnostic assessments and mental health services. In order to enter case management, the child must have a diagnostic assessment completed and/or have a statement of severe emotional disturbance. If at the end of the “assessment period” there is not a diagnosis/statement of severe emotional disturbance, CMH would likely refer to the Early Childhood Family Support Coach, various family therapies (attachment therapy, play therapy, parent-child interaction therapy, parenting education, family psychoeducation, etc., as the challenges often stem from an issue with the home environment or family system. If there is a developmental disability the case is referred to waivered services/DD case management.
